• Indicate whether you are appearing as a Regular or Repeat candidate.
• Select your Programme (UG, PG, Ph.D.) and fill in the Discipline/Subject and Semester clearly.
• Choose your Stream (B.A., B.Sc., B.Com, M.A., M.Sc., M.Com, Others) and specify the Session.
• Fill in your Roll No. (if applicable) Registration No. and Registration Year.
• Fill in  your personal as well as correspondence information in details
• Fill in your subject details, i.e. Course Code, Course Title, Type of Course (Core/Minor/etc.) and the Credits for each subject.
• Read the declaration statement carefully.
• Sign and date the declaration to confirm that all information is accurate.
• Review all entries for completeness and accuracy before submission.
• Submit the form to office of the controller of examinations of the university and retain a copy for your records.
• Seek assistance from your department or institution if you have any questions.
• The payment receipt is attached to the exam form, along with all relevant documents, including your previous semester’s mark sheet, passport photograph, and registration certificate etc.
